This app is for high-school aged students with disabilities with an IEP, ages 14 and older. An IEP (Individualized Education Program) details your special education services. After completing a brief 10 minute survey, the app produces an individualized “Draft Transition Plan,” based on your answers. A “Transition Plan” is an important part of your IEP document. A “Transition Plan” is your road map to transition to adult life after high school. You can then email a copy of your draft transition plan to yourself or others.

Your school must develop a written transition plan as part of your Individualized Education Program (IEP). Having an effective transition plan is important. It helps you get the right services and supports so you can do what you want after high school. A transition plan can help you do things like get a job, train for a job, continue your education, and live independently.
